Liability Release
I hereby release the Accelerate: Dental Revenue Workshop, National Electronic Attachment, Inc., dba Vyne Dental( “Vyne Dental”), and its officers, employees and agents, and any other people officially connected with this event, from any and all liability for damage to or loss of personal property, sickness or injury from whatever source, legal entanglements, imprisonment, death, or loss of money, which might occur while participating in this event. Specifically, I release Vyne Dental from liability or responsibility for (my physical condition, and for the presence or actions of any other participants).

Photograph & Video Release
I hereby authorize Vyne Dental to publish photographs taken of me at the Accelerate: Dental Revenue Workshop on 5/9/2024 & 5/10/2024 for use in Vyne Dental’s print, online and video-based marketing materials, as well as other publications Vyne Dental contributes to from time to time.

I hereby release and hold harmless the Vyne DentalⓇ Accelerate: Dental Revenue Workshop from any reasonable expectation of privacy or confidentiality associated with the images specified above. I understand that participation in this event is strictly voluntary and I freely choose to participate. I will not receive financial compensation for photo usage. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.

You agree to receive marketing information from Vyne Dental. You may opt out of receiving any marketing information at a later date. You have the right at any time to opt-out.

Force Majeure
In the event Vyne Dental must cancel or postpone this Event due to unforeseen related circumstances, including but not limited to a pandemic or epidemic, Vyne Dental will determine a refund of a percentage of the Registration fee.

Attendee Information
Your registration information will not be distributed prior to the workshop. After the workshop, certain information will be disseminated to attending sponsors only: name, title, company name.

Cancellation Policy
I understand that cancellation of the Vyne Dental Accelerate: Dental Revenue Workshop is possible, and that there are costs associated with the set-up and other expenses of the event. I agree to the cancellation policy: Registration tickets are refundable for 24 hours after registration minus a $75 administration fee. Registration Tickets will be 50% refundable within 30 days of the event or (4/8/2024). Inside 30 days of the event (4/7/2024) no refund will be available. If possible, Vyne Dental will offer a complimentary registration to the next iteration of their Accelerate: Dental Revenue Workshop. The Accelerate: Dental Revenue Workshop reserves the right to refuse admission.
